Minimum Requirements
Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Wisconsin Physical Therapy license required to start the assignment.
BLS certification from the American Heart Association or American Red Cross may be required.
Experience working in a skilled nursing facility preferred.
Client requirement: ability to manage caseloads in a SNF setting and provide skilled physical therapy services per facility documentation and regulatory standards.
Preferred: experience with 5x8 schedules and documentation systems commonly used in post-acute care.

Assignment Details
Facility Type: Skilled Nursing Facility.
Shift Name: Days.
Shift Details: Days 08:30–16:30.
Work Schedule: 5 shifts per week, 8 hours per shift.
