Hospital Clinical Pharmacist - Patient-Centric Care

Beverly Hospital is seeking experienced and committed Clinical Pharmacists to join our team. Beverly Hospital is a Level III Trauma Center and not-for-profit community hospital. Located on the North Shore of Massachusetts, the hospital is part of the renowned Beth Israel Lahey Health System and utilizes the integrated resources to provide high quality care for our patients, including for neonatal, pediatric, adult and geriatric patients. Inpatient maternity and psychiatry services are also offered for the community. The Pharmacy Department provides services to the Northeast Hospital Corporation which is comprised of Beverly Hospital, Addison Gilbert Hospital, BayRidge Hospital and Beth Israel Lahey Heath Care Center-Danvers. Clinical Pharmacists ensure safe, effective, timely, patient-centered and evidence-based care. Pharmacists are integral to the healthcare team and rotate through decentralized and centralized shifts. The role requires interpreting and evaluating medication orders and prescriptions, and providing medication expertise within the interdisciplinary team. Pharmacists prepare and dispense medications in accordance with established hospital policies and laws governing such acts. Pharmacists supervise and verify the accuracy of all work performed by pharmacy technicians/interns. Pharmacists at Beverly are involved with various initiatives ranging from quality improvement, to safety and operations. Additionally, pharmacists serve on interdisciplinary committees and as liaisons across the institution. All members of the Pharmacy team are responsible for developing and promoting collaborative and collegial relationships within the department and with nurses, physicians and interdepartmental personnel as they relate to the pharmaceutical care of the patients. Job Description: Essential Duties & Responsibilities including but not limited to:

1. Clinically reviews all medication orders to ensure that medications entered into the patient's profiles are safe for administration. Assessment should include, appropriate drug selection, medication allergy, dose, route of administration, diagnosis, relevant laboratory results, therapeutic duplication, drug interactions and age of patient.
2. Intervenes with appropriate health care professional when necessary, and documents clinical interventions consistent with departmental policy.
3. Appropriately enters reviewed orders into the Pharmacy Information system with attention being paid to how this medication will be administered by the nurse.
4. Supervises pharmacy technician and interns to ensure that the daily needs of our patients are being met. Educate technician and interns as to safe medication practices.
5. Provides drug information to medical staff and patients upon request or when need is apparent. Documents request into the intervention database in accordance with departmental policy.
6. Responsible for day-to-day management of drug distribution, including all controlled substances, IV medications, extemporaneously compounded medications, repackaged medications and investigational drugs.
7. Responsible for understanding and utilizing all technology to accomplish the above.
8. Serves as a liaison between pharmacy department and any health care provider.
9. Pharmacists are responsible for their own professional development including, meeting all requirements for continuing education, keeping abreast of current professional literature, both from a clinical and medication safety perspective.
10. Actively participates in continuous performance improvement activities of the department, including providing suggestions on how to improve day-to-day operations.
11. Performs assigned projects in an efficient and timely manner.

Minimum Qualifications:

SKILLS & COMPETENCIES REQUIRED:

• Knowledge of hospital practice and operations
• Strong written and verbal communication skills.
• Ability to interact constructively with supervisors, peers, and other colleagues
• Expertise in clinical knowledge and patient care

LICENSES, CERTIFICATIONS, and/or REGISTRATIONS: • Current licensure as a pharmacist in the Commonwealth of Massachusetts required

EDUCATION:

• Graduate of an ACPE accredited College of Pharmacy required

EXPERIENCE:

• Clinical pharmacy experience (2+ years) in a hospital setting and/or completion of a Pharmacy Practice Residency preferred.
• Exceptional aptitude or knowledge may be considered in lieu of experience.

Pay Range: $57.00 - $76.71 The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
