- Conduct in-depth evaluations of patients in both inpatient and outpatient contexts to create personalized treatment plans
- Implement physical therapy interventions that focus on enhancing mobility, strength, balance, and overall function
- Work closely with a cohesive team of nurses, physicians, case managers, and rehabilitation professionals to maximize treatment outcomes
- Inform and guide patients and their families regarding therapeutic objectives, discharge planning, and exercises for home care
- Maintain clear and thorough documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory standards
- Engage in care team discussions and interdisciplinary meetings to ensure a unified approach to patient care
- Doctorate in Physical Therapy (DPT) or a corresponding degree from an accredited institution
- State licensure as a Physical Therapist
- CPR certification is preferred
- Specialty certifications (e.g., in neurology or geriatrics) are beneficial but not necessarily required
- Preferred experience in a similar environment
- Capability to address patients with complex medical and mobility needs
- Exemplary communication and collaboration skills
- Proficiency in digital charting and clinical documentation systems
