Description
  • Conduct in-depth evaluations of patients in both inpatient and outpatient contexts to create personalized treatment plans
  • Implement physical therapy interventions that focus on enhancing mobility, strength, balance, and overall function
  • Work closely with a cohesive team of nurses, physicians, case managers, and rehabilitation professionals to maximize treatment outcomes
  • Inform and guide patients and their families regarding therapeutic objectives, discharge planning, and exercises for home care
  • Maintain clear and thorough documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory standards
  • Engage in care team discussions and interdisciplinary meetings to ensure a unified approach to patient care
Education
  • Doctorate in Physical Therapy (DPT) or a corresponding degree from an accredited institution
Certifications
  • State licensure as a Physical Therapist
  • CPR certification is preferred
  • Specialty certifications (e.g., in neurology or geriatrics) are beneficial but not necessarily required
Skills
  • Preferred experience in a similar environment
