Registered Nurse – Home Health
Schedule: Monday–Friday, 8:30 AM–5:00 PM
On-Call: 6 days/month, including 2 weekend days and 4 weekdays
Productivity: 30 units per 40-hour week
Mileage: $0.43/mile
Service Area: Chatham, Liberty, Bryan & Effingham Counties, GA
Position Overview
We are seeking an experienced Registered Nurse (RN) to provide high-quality home health care, coordinate individualized care plans, and support positive patient outcomes throughout the community.
Responsibilities
- Perform comprehensive patient assessments and develop individualized care plans.
- Provide skilled nursing care and monitor patient progress and outcomes.
- Coordinate care with physicians, social workers, discharge planners, families, and other healthcare professionals.
- Manage care transitions and connect patients with appropriate support services.
- Educate patients and families on treatment plans, disease management, and prevention.
- Maintain accurate and timely clinical documentation.
- Identify opportunities to prevent adverse outcomes and promote overall health.
- Advocate for patient privacy, safety, and quality of care.
Requirements
- Unencumbered Georgia RN License required.
- Home health experience required.
- Proficiency with OASIS documentation and accuracy .
- Experience using HCHB (Home Care Home Base) .
- Associate's or Bachelor's Degree in Nursing or related field.
- Strong clinical judgment, communication, and organizational skills.
- Case Management certification preferred.
