Position Summary

We are looking for a Physical Therapist who is passionate about providing high-quality, patient-centered care and helping residents improve mobility, strength, physical function, independence, and overall quality of life.

Responsibilities

• Assess patients' physical abilities and limitations through comprehensive evaluations
• Develop individualized treatment plans based on patient needs, goals, and clinical findings, incorporating evidence-based practices
• Implement therapeutic exercises, functional activities, and appropriate modalities to improve mobility, strength, balance, and overall function
• Educate patients and their families on treatment plans, home exercises, safety strategies, and self-management techniques
• Monitor patient progress and modify treatment plans as appropriate
• Collaborate with interdisciplinary team members to ensure comprehensive patient care and effective discharge planning
• Maintain accurate and timely documentation of evaluations, treatment interventions, patient progress, and outcomes in compliance with regulatory standards
• Participate in care planning and interdisciplinary meetings as needed
• Stay current with advancements in physical therapy practices and evidence-based treatment approaches
• Adhere to all applicable state, federal, facility, and professional standards

Qualifications

• Master's (MPT) or Doctor of Physical Therapy (DPT) degree from an accredited program
• Valid state licensure or eligibility for licensure as a Physical Therapist
• Strong knowledge of medical terminology and physical therapy practices
• Experience in a skilled nursing setting preferred but not required
• Familiarity with discharge planning processes to facilitate smooth transitions for patients
• Experience treating geriatric populations is a plus
• Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals
• Strong organizational and time-management skills
• Compassionate, dependable, and patient-focused approach to care
