Remote Integrated Case Manager RN (Virginia)

Sentara Health Administration, Inc. is seeking a Remote Integrated Case Manager. The role involves telephonic and face-to-face assessments, developing and monitoring care plans, and coordinating with health care teams to optimize patient care.
