Job Title: Contingent Clinical Care LVN/LPN PT - MC Address: St. Peters, MO Job Description: Utilize advanced clinical expertise to deliver high-quality, evidence-based patient care. Assess patient needs, administer treatments and medications, and conduct routine health assessments. Monitor vital signs, evaluate complex patient conditions, and document treatment responses. Manage patient access to healthcare and connect individuals with community resources for medication, self-care, and disease management. Conduct medication reviews to identify potential side effects and drug interactions. Develop and administer mental health assessments to screen for depression and evaluate mood and cognitive function. Critical Information: Work schedule will be based on clinic needs, typically between 7-10 hours, with shifts created in 6-week blocks following training. Requires onsite presence as needed. Remote work is not applicable. Role involves compliance with safety and healthcare standards. Education/Licenses Needed: Active and unrestricted license as a Licensed Practical or Vocational Nurse in the United States.
