St. Marys Center is a community of hope, healing, and justice dedicated to serving older adults, preschool families, and individuals experiencing homelessness in Oakland and the surrounding Alameda County community. St. Marys Centeroperatestransitional housing, a senior center, and preschool and is developing permanent supportive housing as part of a comprehensive, collaborative community development strategy along the San Pablo Avenue corridor. Community Services Program Overview
The Community Outreach & Services (COS) Department helps seniors remain safely housed and socially connected while reducing isolation, improving wellness, and promoting access to community-basedsupports. COS programming includes congregate meals, wellness activities, fall prevention, computer education, health navigation, case management support, behavioral health education, and Friendly Visiting services.
Position Summary
The Community Services Supervisor provides day-to-day leadership and oversight of the Community Outreach & Services (COS) Department and the staff who deliver programming at St. Marys Centers Community Center. Reporting to the Director of Community Services & Partnerships, this position supervises the COS team;including Case Managers, the Friendly Visiting Coordinator, the Senior Center Activities Coordinator, the Primary Prevention Specialist, and the Community Health Worker;and ensures that programs operate efficiently, meet contract and grant requirements, and reflect the mission and values of the organization.
The Supervisoris responsible forcoordinating schedules and workflows,monitoringprogram quality and outcomes,and ensuringaccuratedocumentation and compliance across the department. This position partners closely with the Director on strategic planning, program design, budget monitoring, and continuous quality improvement, and serves as a key liaison to participants, volunteers, community partners, and Alameda County agencies.
This position also supports Medi-Cal Administrative Activities (MAA/CMAA) through oversight of eligible outreach, referral coordination, benefits navigation, health-related case management activities, and documentation that improves access to medical, behavioral health, and supportive services.
Essential Duties & Responsibilities
Staff Supervision & Leadership
- Superviseand support COS staff, including Case Managers, the Friendly Visiting Coordinator, the Senior Center ActivitiesCoordinator, the Primary Prevention Specialist, and the Community Health Worker.
- Provide day-to-day leadership that ensures programs and the Community Centeroperateefficiently, safely, and in alignment with program goals.
- Coordinate staffschedules,caseload distribution,and departmentcoveragetomaintainconsistent program operationsin partnership with the Director.
- Addressstaffperformance and/orconduct concerns in partnership with the Director and Human Resources.
- Foster a positive, respectful, trauma-informed, and inclusive team culture.
Program Operations & Quality Assurance
- Oversee daily operations of COS programs and Community Center activities to ensure efficient, high-quality, person-centered service delivery.
- Identifyoperational barriers and implement workflow improvements that strengthen program efficiency and participant experience.
- Support emergency preparedness and emergency response planning and activities for the Community Center.
- Coordinate facilities, supplies, technology, and resources needed for program operations in partnership withappropriate staff.
Reporting, Data & Compliance
- Monitor data collection and documentation practices to ensure accuracy, timeliness, and completeness across the department.
- Ensure documentation and program activitiescomply withHIPAA,state, county, city, and agency standards.
- Oversee documentation in agency and County systems and support audits, quality assurance reviews, and corrective action when needed.
- Support MAA/CMAA documentation and reporting aligned with county and state requirements for eligible activities.
Strategic Planning & Program Development
- Provide input onprogram design, service models, staffing structure,continuous quality improvement,and resource needs based on participant and community trendsin partnership with the Director.
- Assistwithidentifyingresourceand funding needs for COS programs.
- Recommend and help implement new or expanded programming that responds to the needs of older adults and the surrounding community.
Collaboration & Community Partnerships
- Collaborate withHousing Services, Behavioral Health, Community Center staff, and leadership to coordinate services and care.
- Partner with Alameda County agencies, hospitals, clinics, managed care plans, and community-based organizations.
- Engage participants, caregivers, volunteers, and family members to support coordinated, person-centered services.
- Support outreach events, presentations, and community engagement activities that promote COS programs.
Direct Service Support
- Provide back-up case management, crisis intervention, and de-escalation support withinscopeof practice as needed.
- Identifyand report suspected elder abuse or neglectin accordance withmandated reporting requirements.
- Step in to support front desk, congregate meal services, workshops, and program activities when coverageis needed.
- Provide referrals and linkage to healthcare, behavioral health, nutrition, housing, and social services.
Qualifications
- Associates degree in Social Work, Human Services, Public Health, Gerontology,Sociologyor related field.Bachelors degree preferred.
- Minimum of 3 years of experience working with older adults, vulnerable populations, or community-based programs, including at least 12 years of supervisory or program coordination experience.
- Demonstrated ability to lead, coach, and develop staff and to manage day-to-day program operations.
- Strong understanding of case management, aging-related issues, social isolation, behavioral health, and chronic disease management.
- Knowledge of community resources, benefits systems, and supportive housing.
- Familiarity with Medi-Cal, MAA/CMAA, SSI, IHSS, and CalFresh.
- Knowledge of trauma-informed care, motivational interviewing, harm reduction, elder abuse prevention, and mandated reporting.
- Experience running reports and using data tomonitorprogram quality and outcomes.
- Strong organizational, communication, interpersonal, and documentation skills.
- Proficiencywith computers and database/documentation systems.
- California Drivers License.
- Bilingual in Cantonese, Mandarin, or Spanish a plus.
Work Environment
This positionoperatesprimarily at St. Marys Centers Community Center and in community-based settings including participant homes, senior housing sites, community centers, and partner organizations throughout Alameda County.
