Wound Care Nurse (RN / LPN) – Sister Facilities

Facilities: St. Luke North Canton & St. Luke Portage Lakes (SNF / ALF)

Pay: Starting at $40.00 / hour

Job Type: Full-time

St. Luke is seeking a dedicated, detail-oriented Wound Care Nurse to manage and oversee wound care operations across our sister facilities: St. Luke North Canton and St. Luke Portage Lakes. In this dual-facility role, you will champion skin integrity and wound healing for our Skilled Nursing Facility (SNF) and Assisted Living Facility (ALF) residents.

• Specialized Care: Assess, stage, and treat pressure ulcers, surgical wounds, diabetic ulcers, and other skin integrity issues across both the North Canton and Portage Lakes campuses.

• Treatment Coordination: Administer wound care treatments, dressings, and therapies according to physician orders and established clinical protocols.

• Collaboration & Documentation: Work closely with physicians, directors of nursing, and floor staff to update care plans. Maintain meticulous electronic documentation of wound progression.

• Prevention & Education: Educate nursing staff, residents, and family members on proper positioning, skin care techniques, and preventive measures.

• Rounds: Conduct regular wound rounds and participate in quality assurance reviews to ensure optimal resident outcomes.

• Active, unencumbered RN or LPN license in the state of Ohio.

• Previous experience in wound care management is highly preferred (WCC, WOCN, or CWS certification is a major plus, but not required).

• Prior clinical experience in a Skilled Nursing (SNF) or Assisted Living (ALF) environment.

• Ability to travel reliably between our North Canton and Portage Lakes facilities.

• Strong communication, assessment, and time-management skills.
