Signify Health, part of CVS Health, is seeking a Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations for patients in their homes, bridging gaps in care. The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and potential diagnostic tests ordered by the health plan.
