The  Field Supervisor plays a critical role in upholding the agency’s mission by promoting its core values, administrative policies, and procedures to ensure the highest quality of care is consistently delivered to our clients. Working within a healthcare setting and across various  Personal Health Care (PHC) Programs —both  Waiver and Non-Waiver —the Field Supervisor is expected to demonstrate professionalism, ongoing competency, and a commitment to continuous improvement.

Key responsibilities include:

• Ensure to service all clients their  rights and responsibilities.
• Ensuring that  all clients receive consistent and appropriate services as required by agency standards.
• Identify and report all significant changes in the client’s health and living conditions and knowing when to request an increase/decrease of hours, tasks, and service program.
• Responsible for monitoring clients that are in service interruptions, no attendants, service breaks, out of town, suspensions, hospitalizations, etc.
• Actively recruiting and promoting the onboarding of new attendants to ensure continuous service delivery for clients in need of ongoing care.
• Providing  supervision and support to both clients and attendants in alignment with agency policy and client needs.
• Conducting and maintaining  annual assessments, supervisory visits , and  attendant orientations .
• Reporting: Provide weekly service status updates on all clients in the region during scheduled regional meetings.
• Meeting Attendance: Attend all scheduled mandatory meetings unless prior arrangements are made with you Supervisor.

This role requires proactive oversight, effective communication, and a deep understanding of regulatory requirements and best practices in client care. The Field Supervisor also serves as a vital link between the agency, providers, and clients, ensuring quality, compliance, and continuity of care.

Responsibilities and Duties include, but not limited to:

• Maintain a positive work relationship with clients, employees, and attendants.
• Agency’s devices must be used to collect documentation of attendant ID’s done in the field.
• Process new hire(s) of attendants in the field and/or office.
• Perform Competency Evaluation and Orientation to new attendant hires.
• Conduct annual performance evaluation for all attendants within the region and turned into HR department by the 5th of the following month.
• Responsible to  scan daily and  turn in all HR documents to HR Department; the following day of the 1st working day or completion of the employee packet.
• Required to be out in the field most of the month to conduct and complete all monthly visits.
• Responsible for all clients and attendants in the region, regardless of assigned area.
• Perform all assigned tasks in compliance with the agency’s policies and procedures.
• Maintain and supervise all aspects of client quality of care as per ISP.
• Responsible to defuse and resolve an issue between an attendant and a client.
• Responsible to schedule appointments for client visits.
• Address and report any complaints voiced by client or attendant to Administrator, Alternate Administrator and Human Resource Manager.
• Annuals, Supervisory Visits, SOC’s, 211’s and Client Satisfaction Surveys are be maintained current and in compliance.
• Responsible for the supervision of all home care attendants (providers).
• Responsible to ensure that all attendants are performing all assigned, allowable tasks & schedule according to client’s service plan.
• Assist (100% of the time) client coordinators in finding attendants for the region primarily the area assigned.
• Ensure that all paperwork is completed before submitting to QA department.
• Must be able to assist with QA documents and charts of all clients and employees.
• Responsible for turning in all paperwork by the 7th day of the initial visit for SOC, AOs and SV’s.
• Assigned monthly paperwork must ALL be submitted by the end of the month.
• Must submit paperwork through sign-in sheet to ensure paperwork is not lost.
• Employee needs to be able to work within a high pace environment, servicing 250+ clients and attendants.

Qualifications:

• Two years of full-time study at an accredited college or a university in a health-related field or one year of supervisory experience in a health-related setting for every year of required year of college
• One year of experience and/or training in the care of individuals with functional limitations
• Must have at least Two (2) years of experience in Personal Assistance Services Program
• Computer Literacy

Conditions:

• Be able to read, write and comprehend English.
• Not have been convicted of a felony or misdemeanor listed Chapter 99 (relating to Criminal Convictions Barring Facility Licensure).
• Be able to communicate, comprehend, and translate Spanish when necessary.
• Be highly organized, responsible, and dependable, multi-task and work independently.
• Have knowledge of medical terminology.
• Must have excellent communication skills and telephone etiquette.
• Valid Driver’s License. Drive 80% of scheduled time to conduct home visits.

Job Type: Full-time

Pay: $18.00 - $24.00 per hour
