Social Worker (LBSW) Specialty: Client Duration: 13 weeks Shift: Days Hours per Shift: 8 hours variable – Monday & Tuesday (could be adjusted if needed) Experience: 2 years Social Worker Experience, Dialysis Experience License: Active Michigan License Certifications: Active American Heart Association BLS Must-Have: - Work History - Skills Checklist, current to 1 year, pertinent to the specialty (Social Worker) - 2 Supervisory Reference Forms, taken within the last two years Description: Helps patients and their families manage the emotional, social, and practical challenges of chronic kidney disease and treatment. Provider assists in navigating the complexities of dialysis. Facilitates support services and resources tailored to individual patient needs. Works collaboratively with healthcare teams to advocate for patient care. Provides counseling and emotional support to enhance wellness and resilience. Ensures patients understand their treatment options and plan of care.
