Inpatient Case Manager

This position involves working within an inpatient setting to manage patient care effectively. The primary focus is to facilitate the transition of patients from remote care to in-person care, ensuring their needs are met throughout their stay.

Responsibilities

• Coordinate patient care activities across multiple locations.
• Initiate and manage remote care for patients prior to their in-person visits.
• Assess patient care needs and develop care plans accordingly.
• Collaborate with healthcare professionals to optimize patient outcomes.
• Ensure compliance with healthcare regulations and standards.

Qualifications

• Current Basic Life Support (BLS) certification.
• Experience in case management or a related field is preferred.
• Strong communication and organizational skills.

Work Schedule

This position requires an 8-hour shift from 2:30 PM to 11:00 PM.

Important Information

This position may require orientation at a different location before transitioning to in-person duties.
