Registered Nurse - Case Manager

Base pay range $55.00/hr - $60.00/hr Duties Assess patients, coordinate care, and plan discharges. Organize and participate in family meetings. Contribute to the development of treatment plans. Arrange DME, SNF, and ambulance transport independently. Utilize computerized documentation systems. Work with HMOs, IPAs, and understand adoption/foster care, drug/alcohol abuse treatment, mental health crises, and protective services.
