What You'll Do

• Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders within a Long Term Care setting
• Develop and implement individualized treatment plans based on assessment results
• Document patient progress, treatment outcomes, and discharge summaries in compliance with regulatory standards
• Collaborate with healthcare team members to coordinate care and optimize patient outcomes
• Provide consultation and counseling to patients, families, and caregivers
• Support and supervise CFYs or student speech-language pathologists as needed
• Maintain all documentation accurately and participate in required meetings and trainings

What You Need

• Active ASHA certification and State Licensure in Speech-Language Pathology
• Prior experience in Long Term Care (preferred but not required)
• Strong communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
