Job Description Summary The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with policies, professional standards, and regulatory entities. This entry‑level position allows the pharmacist to coordinate patient services with the Nursing Department, monitor progress, and educate patients, caregivers, and staff on medication use. Job Responsibilities Determine the suitability of individual patients for home care, collecting and organizing patient‑specific information, and assessing medical condition and therapy suitability. Design, recommend, monitor, and evaluate patient‑specific pharmacy care plans. Modify plans when appropriate based on evaluation. Collect information to prevent, detect, and resolve medication‑related problems, including identifying monitoring parameters and performing nutritional assessments. Provide medication‑use education to staff, other professionals, patients, and caregivers using effective techniques. Ensure continuity of pharmaceutical care to and from home and other settings, communicating pertinent information systematically. Prepare and dispense medications following policies, calibrate equipment, check concentrations, avoid incompatibilities, ensure stability, and adhere to safety and quality assurance practices. Prepare appropriate labels, inspect final medication, and comply with state/federal laws, USP standards, DEA regulations, and inventory policies. Manage medication administration and related equipment, troubleshoot issues, and report incidents and errors. Complete QAR documents, monitor inventory discrepancies, and provide timely drug information responses based on literature analysis. Attend educational seminars or CEU opportunities to maintain competency and comply with state CEU requirements. Manage investigational drug products per regulations and company policies. Utilize pharmacy support personnel, prioritize workload, ensure accuracy, and maintain confidentiality of patient and proprietary information. Basic Education and/or Experience Requirements Graduate of an accredited School of Pharmacy. Current, active Pharmacy license as granted by the State Board of Pharmacy. At least one of the following: 1 year of experience in sterile IV admixture procedures. Knowledge of non‑sterile compounding, immunizations, and medication therapy management (MTM). Completion of an ASHP accredited Pharmacy Residency Program. Completion of an Advanced Pharmacy Practice Experience with an accredited school of pharmacy. Equivalent experience approved by the Vice President of Pharmacy or Senior Vice President of Clinical Services. Basic Qualifications Experience providing customer service to internal and external customers, meeting quality standards and evaluating satisfaction. Basic skill in Microsoft Word: opening, editing, formatting, reviewing, printing. Basic skill in Microsoft Excel: formatting, copy/paste, printing. Basic skill in Microsoft PowerPoint: inserting, editing, printing, and slide navigation. Preferred Qualifications & Interests (PQIs) Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience. Experience in home care, hospital, or clean room setting. At least 1 year of indirect management experience of team members, including development, training, and assignment of work. Travel Requirements Willing to travel up to 5% of the time for business purposes within state.
