Certified Medical Assistant

Description Connect Care Management is seeking a compassionate and motivated Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), or Registered Nurse (RN) to join our growing team in Thomson, Georgia. This role focuses on helping patients successfully manage their health from home through care coordination, chronic disease management, and ongoing patient support. The majority of patient interactions are conducted by phone, making strong communication and relationship-building skills essential. Responsibilities Manage and coordinate patient hospital discharges and transitions of care Support medication adherence and patient education Provide Chronic Care Management (CCM) services Monitor and document patient blood pressure and blood sugar readings Communicate regularly with patients to assess needs and provide support Collaborate with providers and care team members to improve patient outcomes Maintain accurate and timely documentation in accordance with company policies Qualifications Current CMA, LPN, or RN credential/license Excellent verbal communication and interpersonal skills Comfortable speaking with patients by phone throughout the day Ability to build rapport and maintain positive patient relationships Strong organizational and documentation skills Healthcare experience preferred; 2+ years of experience is preferred, but qualified candidates with less experience will be considered Work Location: Thomson, GA.
