Key Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans
• Provide skilled occupational therapy services to residents in a skilled nursing setting
• Document patient progress, treatment goals, and outcomes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary team members to coordinate patient care
• Educate patients and caregivers on therapeutic techniques and home exercise programs

Required Qualifications

• Active Occupational Therapist (OT) license in the state of Illinois
• Graduate of an accredited Occupational Therapy program
• Ability to work day shift hours

Preferred Experience / Skills

• Prior experience in a skilled nursing facility or long-term care setting
• Experience with geriatric patient populations
