Job Description We are seeking a qualified Physical Therapist to join our team. The position involves providing therapy services to clients in a home health setting across designated territories, which include Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington, as well as additional areas in Oakland, Berkeley, or Vallejo, as needed. Responsibilities include evaluations, treatment planning, and implementing therapy interventions for patients.

Responsibilities Conduct thorough evaluations and assessments of patients. Develop and implement individualized treatment plans based on patient needs. Provide therapy interventions as per care plans. Complete documentation in accordance with agency and regulatory requirements. Deliver care to patients with various health conditions, including Covid+ and PUI patients. Participate in ongoing education and training as required.

Required Experience / Certifications / Licensure Current California Physical Therapist License. Basic Life Support (BLS) certification. Valid Driver's License and auto insurance. Required immunizations and health screenings (TB, MMR, Varicella, HepB, TDap, Annual Flu). Drug screening and physical examination upon hire.
