Key Responsibilities\n

  • Facilitate direct clinical support by conducting individual, group, and family therapy, focusing on those experiencing major mental illnesses or severe emotional disorders.

  • Develop and implement integrated care plans that coordinate access to services for clients, their families, and other healthcare providers.

  • Participate actively in team meetings, contributing to collective goals and fostering collaboration among multidisciplinary teams.

  • Conduct thorough assessments and develop evidence-based treatment plans, using appropriate screening tools tailored to the developmental needs of diverse populations.

  • Manage a caseload effectively, ensuring that client treatment plans are responsive to immediate needs while promoting empowerment and positive outcomes.

  • Assist with crisis intervention efforts and provide ongoing support, ensuring safety and adherence to ethical guidelines.

  • Engage in quality improvement initiatives, participating in committees aimed at enhancing agency service delivery.

  • Monitor client progress, manage admissions and discharges, and maintain accurate documentation of services rendered.

  • Provide on-call coverage as necessary, supporting clients during emergencies or urgent situations.

Patient Population / Client Focus\n

The focus of your practice will be on serving clients at our Hospice Center, where you will work with individuals facing terminal illnesses. Sensitivity to the emotional and psychological needs of clients and their families is paramount, as you will help them navigate difficult discussions and support them in making informed decisions about their care and treatment. An important aspect of this role is to foster a supportive environment in which clients can express their fears, hopes, and decisions concerning end-of-life care.

  • A Master’s degree in Social Work from an accredited program and current LCSW licensure.

  • Strong knowledge of mental health assessments, risk assessments, treatment planning, and crisis management.

Compensation and Benefits\n

The salary for this position ranges between USD 70,000 and 85,000 annually, commensurate with experience and qualifications.
