Occupational Therapist Position

Provide specialized occupational therapy services to patients in a skilled nursing facility, helping improve their functional independence and quality of life. This role involves evaluating patient needs, developing treatment plans, and collaborating with healthcare teams.

Responsibilities:

• Evaluate patients' clinical and functional status to determine therapy needs.
• Develop and implement personalized treatment plans aligned with patient goals.
• Collaborate with interdisciplinary teams to coordinate care and optimize outcomes.
• Document patient progress and maintain accurate records in Net Health or equivalent system.
• Adapt therapy approaches based on patient response and changing needs.
• Ensure compliance with facility standards and state licensing requirements.

Qualifications:

• Hold an active California Occupational Therapist license (or in process under the 60-day rule).
• Possess SNF (Skilled Nursing Facility) experience; new graduates are welcome with mentorship.
• Demonstrate strong documentation and time-management skills.
• Maintain a flexible and collaborative approach to team efforts.
• Prioritize patient-centered care with compassion and professionalism.
• Have the ability to float to nearby facilities as needed.
