Job Description
JOB COMMENTS/REQUIREMENTS: 0900-2130, every other Saturday, rotating overnight Sunday call. BLS and ACLS required.

· Candidates must be able to start on the listed start date, or no more than one week later
· RTO must be on the cover page and cannot exceed 7 days. No RTO during the first week of assignment

· All shifts the candidate is willing to work
· All departments the candidate is experienced and willing to work

Submission Requirements
· Must have a minimum of 24 months Dialysis experience (with recent experience in last 12 months) - [REQUIRED]
· Life support certifications must be from AHA, ARC, or ASHI - [REQUIRED]
· No hits on state license to practice - [REQUIRED] [DOCUMENT]
· RTO cannot exceed 7 days, No RTO during the first week of assignment - [REQUIRED]
· Candidate driver’s license to verify residency - [REQUIRED]
· Travel experience - [HIGHLY PREFERRED]


Active RN License
