Licensed Practical Nurse (LPN) Day Shift Private Duty Part-Time 30 hours

Private hire LPN opportunity to work with a 32-year-old female client at her home in Warwick, New York. Position Part‑time day shift, 30 hours per week. Wednesday and Thursday 11 am – 8 pm, and Saturday and Sunday 12 pm – 6 pm. Essential Job Duties Provide personal attendant/companionship services to the client at his residence. Maintain a safe, secure, clean, and orderly environment for the client. Assist with daily routines, including bathing, dressing, grooming, and toileting as needed. Assist with a range of motion exercises and exercise routines established by a physical therapist. Perform light housework when such activities are related to the client’s IADLs and ADL. Assist with medication administration. Attend client appointments during the scheduled shift as needed. Take and log vital signs. Provide stimulation and entertainment, including conversation, reading, television, scheduling activities, outings, etc. Provide support, comfort, and companionship. Complete shift logs thoroughly and accurately for each shift. Assess and report any medical changes. Respond quickly to emergencies by notifying the appropriate persons and calling 911 as needed. Perform other duties as assigned to support the client’s care and companionship. Expectations Maintain regular and consistent attendance. If you are unable to work your scheduled shift or need to make changes to your schedule, you must communicate this directly to the designated person. Limit personal calls, text messages, social media, etc., to breaks or personal time, except in the case of a family emergency or when they relate to the client. Phones should be silent and put away while working, especially with the client and when there are tasks to be completed. Must exhibit professional verbal, written, and interpersonal skills in all interactions. Must demonstrate a high level of autonomy, which may include completing and communicating tasks without being reminded. Provide proactive support to the client. Strong attention to detail. Ability to communicate information concerning the health and condition of the client clearly and concisely. Ability to work with the client in a patient and supportive manner. The employee must notify TEAM immediately if the employee’s driver’s license is suspended or revoked for any reason or if the employee is convicted of any crimes. All hours must be recorded on the TEAM timesheet during each shift. Working outside of the employee’s scheduled hours without advanced approval is prohibited. Physical Ability Lift, carry, and push up to 75 pounds. Work while standing 25‑50% of the time.
