Registered Dietitian
Job Summary:
Tucker House Nursing and Rehabilitation, a premier long-term and post-acute rehabilitation center in Philadelphia, Pennsylvania, is seeking a compassionate and experienced Registered Dietitian to join our dedicated team. Responsibilities:
Conduct initial dietary assessments and develop personalized nutrition plans for residents
Collaborate with healthcare team members to ensure dietary needs are met and addressed
Conduct regular follow-up assessments and adjust nutrition plans as needed
Provide education and guidance to residents, families, and staff on nutrition and wellness
Develop and implement menus and meal plans that cater to residents' dietary needs and preferences
Monitor food intake and track resident progress
Participate in quality improvement initiatives to continuously improve our services and outcomes
