Job Responsibilities:- Provide patient-centered care to individuals in their homes
- Assess patients' functional abilities and develop personalized treatment plans
- Implement therapeutic activities and exercises to improve patients' independence and quality of life
- Educate patients and their families on techniques for managing daily activities
- Collaborate with other healthcare professionals to ensure comprehensive care for patients
- Valid Occupational Therapist license in the state of Nebraska
- Experience working with patients in a home healthcare setting
- Strong interpersonal and communication skills
- Ability to work effectively in a team environment
- Passion for providing patient-centered care
