Responsibilities
- Administer medications and treatments per physician orders, within the scope of practice for an LPN
- Monitor residents’ conditions, perform vital signs, and document findings in the electronic health record
- Conduct head-to-toe assessments and provide essential nursing care to residents
- Assist with activities of daily living and maintain resident safety
- Administer injections and provide routine wound care as directed
- Collaborate with registered nurses and the care team to implement and update care plans

Requirements
- Active LPN license in good standing; eligible to practice in the state of New Hampshire
- Current CPR/BLS certification from a recognized provider
- LPN diploma or certificate from an accredited program
- Experience in a long-term care setting is preferred
- Ability to work in a travel assignment with shift flexibility as assigned
- Willingness to undergo background check and licensure verification
