What You'll Do Perform evaluations, assessments, reassessments, and develop individualized treatment plans Provide skilled physical therapy services to residents in an outpatient, independent living, and assisted living environment Educate residents, families, and team members on therapy techniques and wellness strategies Document clinical services accurately and timely according to departmental standards Demonstrate sound clinical judgment and evidence-based treatment practices Supervise and mentor Physical Therapist Assistants, Rehab Technicians, and therapy students Review PTA documentation and ensure compliance with regulatory requirements Screen residents to identify opportunities for therapy intervention Participate in wellness and rehabilitation programming that promotes resident independence Attend required meetings, training, and continuing education opportunities Contribute to a culture of safety, service excellence, and teamwork Work schedule: 30-40 hours per week.

ESSENTIAL DUTIES

Model a service attitude towards others, take ownership in solving problems and take the initiative to make things better, be friendly and courteous to residents, patients, visitors, and co-workers Ensure resident, patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards Maintain good attendance record Document all assessments, reassessments, progress notes, discharge summaries and other clinical work in the medical record per departmental policy and procedure Provide resident, family and staff education pertaining to Physical Therapy modalities to ensure understanding and proficiency of care Demonstrate good clinical judgment in all aspects of resident treatments in which trained to provide Orients, instructs and directs work activities of Licensed Physical Therapy Assistants, Rehab Techs and Physical Therapy students Reviews documentation written by the Licensed Physical Therapy Assistants Screen residents to determine their potential benefit from physical therapy intervention Develops, directs and/or participates in physical therapy programming for the residents Attends in‑services and other meetings as required on an ongoing basis Maintains continuing education per state licensure requirements Other duties as assigned by the Director of Rehab What We're Looking For Current Physical Therapist license in the state of Texas Experience working with older adults in Independent Living, Assisted Living, Outpatient Therapy, Wellness, or related settings preferred Strong clinical skills and knowledge of therapy regulations and best practices Excellent organizational and time‑management abilities Ability to thrive in a dynamic, resident‑focused environment Basic computer proficiency, including Outlook, Word, and Excel Outstanding customer service and communication skills A genuine passion for helping older adults achieve their highest level of function and independence

MINIMUM QUALIFICATIONS

Licensed Physical Therapist in the current state Knowledge of therapy industry and federal, state and local government regulations Demonstrated ability to organize, prioritize and complete multiple tasks in a dynamic environment Demonstrated efficiency strategies for fast paced environment Experience in IL/AL/OP setting or mature population rehabilitation and wellness needs Basic computer skills, Outlook, Word, Excel Excellent customer service skills Genuine caring for and interest in the care of mature adults, handicapped people and hospice patients Ability to perform Essential Duties Ability to perform work tasks within the Physical Demand Requirements Ability to comply with the Resident Bill of Rights and the Employee Responsibilities Ability to comply with organizational and departmental safety policies and procedures Salary Information $47 - $53 Hourly Wage #J-18808-Ljbffr The Outlook at Windhaven
