Nurse Risk Manager and Infection Preventionist

White Oaks Rehabilitation & Nursing Center is seeking an experienced Nurse Risk Manager for direct hire. The Risk Manager is responsible to mitigate patient health risks and to develop and implement policies and processes to ensure the facility’s safety and accountability. The Risk Manager is obligated to ensure that all occurrences/incidents within the SNF are thoroughly investigated and reported according to DOH and policies and guidelines. Additionally, the Risk Manager will work with interdisciplinary teams to assure the effectiveness of the program, and educate and counsel staff. Responsibilities: • Investigate, Determine, and Report (using Occurrence Logs) all Incidents according to the NYS DOH Incident Reporting manual.Assess liability and financial risks (falls, medication errors, abuse and neglect, hazardous conditions, etc.)Report on all ongoing investigations to Administrator, DNS, and IDTImplement mitigation strategies, assess their effectiveness, and make revisions accordingly.Educate and counsel staff. Run Safety Meetings.Initiate and direct all investigations and notifications to authorities.Issue timely reports to regulatory agencies and authorities.• Represent the facility at all audits, inspections, and investigations by governing authorities. Requirements: • At least 5 years of RN nursing experience. Valid NY RN licenseAt least 2 years of experience in long-term care or Skilled Nursing facilities - a must.
