**Required Experience:** - 2+ years of Intensive Care Unit (ICU) experience **Licensure:** - Active Illinois/IL License (Nursys must state Active) **Certifications:** - Active American Heart Association Basic Life Support (BLS) - Active American Heart Association Advanced Cardiovascular Life Support (ACLS) (required within 30 days of start) **Submission Requirements:** - Candidates' Permanent Address (Facility will not consider local candidates - 50-mile radius restriction) - Work history - Skills Checklist, current to 1 year, pertinent to the specialty (Intensive Care Unit) - 2 Supervisory Reference Forms Completed with dates employed listed **Certifications:** - BLS (AHA) - ACLS (AHA) **State Licenses:** - Illinois
