- Conduct thorough assessments of patients’ mental health needs, creating individualized treatment plans tailored to their specific goals and challenges. \n
- Maintain consistent communication with team members, supervisors, and families to discuss client progress and treatment adjustments, ensuring comprehensive support and transparency. \n
- Prepare and present a structured schedule of appointments and home visits, facilitating effective monitoring of patient engagement and therapeutic progress. \n
- Coordinate in-home visits and data collection efforts, including chart reviews and interviews, to gather relevant information that assists in patient assessment. \n
- Monitor the implementation of treatment plans, ensuring all interventions are clinically appropriate and beneficial. \n
- Participate actively in Interdisciplinary Team Meetings to engage in collaborative discussions regarding patient care strategies. \n
- Document client progress in detailed monthly and quarterly notes, contributing to accurate records that drive treatment effectiveness. \n
- Organize and facilitate training for staff focused on the effective execution of treatment plans and documentation standards. \n
- Conduct follow-up interactions via interviews and home visits to provide ongoing support and address evolving patient needs. \n
- Serve as a liaison with community resources and mental health organizations, fostering partnerships that enhance care delivery. \n
- Assist in the development of training materials and procedures that enhance program quality and staff competence. \n
