Clinical Pharmacy Specialist - PRN - AAMC Luminis Health is hiring a mid‑level Clinical Pharmacy Specialist - PRN - AAMC. The position is full time and is based in Annapolis. Position Objective Responsible for carrying out Clinical Pharmacy services through promoting evidence‑based pharmacotherapy and sound pharmaceutical care by developing medication management protocols and drug therapy policies. Serves as a Clinical Pharmacy resource for the health system. Responsibilities Conduct departmental activities to meet pharmacy goals, objectives and policies. Participate in improvements in interdepartmental policies and procedures supporting strategic goals of the Pharmacy Department and hospitals. Maintain accurate records per departmental, federal and state regulations. Complete core competencies and document clinical activities accurately according to policies and procedures. Support the Investigational Drug Service in daily operations. Serve as medication expert on medical and surgical units and provide time‑sensitive, accurate information for other healthcare providers in acute scenarios. Provide pharmaceutical care, dosing and guideline recommendations to impact patient care and reduce readmissions. Guide drug selection and pharmacotherapy regimens following evidence‑based guidelines. Complete pharmacotherapy drug class reviews, formulary additions/deletions, evidence‑based protocol/order‑set integration, medication safety activities and/or targeted medication expense reductions. Assist with appropriate transition of care services, including medication reconciliation and discharge education of high‑risk patients or those with pertinent medication‑related problems. Act as pharmacy representative on multidisciplinary committees and develop pharmacotherapy‑specific educational programs, protocols, standard operating procedures, and order sets. Educate other healthcare providers, via formal and informal in‑services, on approved treatment guidelines to increase appropriate and cost‑effective medication use in medical and surgical units. Serve as a resource for Pharmacy School students and AAMC Pharmacy Residents. Educational / Experience Requirements Doctor of Pharmacy degree (required) Completion of PGY-1 residency or 2 years of demonstrated clinical experience (required) Completion of PGY-2 residency or board certification in pharmacotherapy (preferred) Required License / Certifications Current Maryland State License Working Conditions & Physical Demands Light work; may exert up to 20 lbs occasionally, up to 10 lbs frequently, or negligible force continuously. Rating: light work. Employees may be exposed to blood‑borne pathogens. Specific vision abilities include close, distance, color, peripheral, depth perception, and focus adjustment. Pay Range

$62.13 – $93.19 USD
