Serio, Dr. Guy, and Dr. Frasca.The group provides comprehensive adult primary care with a strong reputation for high-quality, patient-centered medicine and long-standing community relationships.Collectively, the practice manages a broad spectrum of adult medical conditions, with particular expertise in chronic disease management, preventive care, obesity, hypertension, diabetes, and cardiovascular risk reduction.Position SummaryWe are seeking a compassionate and motivated Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join a busy, well-established internal medicine group practice. The APP will work collaboratively as part of a six-physician team, providing high-quality, patient-centered care within a supportive and collegial clinical environment.This position is ideal for an APP who values continuity of care, collaborative physician partnership, and building long-term relationships with adult patients in a supportive, team-based environment.Key ResponsibilitiesProvide comprehensive primary care services to adult patients under physician collaborationEvaluate, diagnose, and manage acute and chronic medical conditionsManage chronic disease states including diabetes, hypertension, obesity, and cardiovascular risk factorsPerform preventive care visits, annual wellness exams, and Medicare wellness visitsOrder and interpret diagnostic tests and labsPrescribe medications in accordance with New York State regulationsEducate patients on disease management, lifestyle modification, and preventive healthDocument clinical encounters accurately and efficiently in the electronic medical record (EMR)Coordinate care with specialists, hospitals, and ancillary services as neededSupport quality initiatives and evidence-based care standardsQualificationsLicensed Nurse Practitioner in the State of New York (or eligible)National board certification requiredBLS/ACLS/DEA requiredMinimum 1–2 years of experience in internal medicine, primary care, or adult medicine preferred (new graduates with strong clinical rotations will be considered)Strong clinical assessment and diagnostic skillsCommitment to patient-centered, high-quality careAbility to work collaboratively in a physician-led practicePosted Salary RangeUSD $69.55 - USD $93.00 /Hr.
