Founded in 2021 by healthcare leaders with over 30 years of experience, we partner closely with Skilled Nursing and Long-Term Care facilities to elevate care standards, improve outcomes, and reduce hospital readmissions.

Our multispecialty team includes board-certified physicians in Family Medicine and Nephrology, working collaboratively with Advanced Practice Providers to deliver timely, patient-centered care.

Position Overview

We are seeking a dedicated and compassionate Advanced Practice Provider (NP/PA) to provide comprehensive medical care to residents in Skilled Nursing and Long-Term Care facilities, along with structured telemedicine on-call coverage.

This role blends:

• Post-acute and chronic disease care

• Regulatory documentation and interdisciplinary collaboration

• Overnight telemedicine support

The ideal candidate is comfortable practicing independently, managing medically complex geriatric patients, and responding confidently to urgent clinical situations.

Core Responsibilities

Skilled Nursing & Clinical Care

• Diagnose and manage acute and chronic medical conditions

• Develop and implement individualized care plans

• Order and interpret diagnostic tests and labs

• Prescribe and manage medications within scope of practice

• Provide post-hospital follow-up care

• Address cognitive, behavioral, and functional changes

• Support quality initiatives aimed at reducing hospital readmissions

• Ensure compliance with CMS and state regulations

• Maintain timely and accurate EMR documentation

On-Call & Overnight Coverage Schedule:

• M/T/Th/F 10Hr Shift 10pm-6am

• Week 1: Saturday/Sunday 8 AM-10:30PM

• Week 2: Monday/Tuesday/Friday: 6PM-8AM

• Week 3: Wednesday/Thursday: 6PM-8AM, Saturday/Sunday 8 AM-10:30PM

• Week 4: off

On-Call Duties Include:

• Respond promptly to patient and facility calls

• Assess, triage, and manage patient care needs via telemedicine

• Order diagnostics and prescribe medications as appropriate

• Coordinate care with facility staff and collaborating physicians

• Document all patient interactions accurately and timely

• Provide professional, compassionate overnight clinical support

Qualifications

Required

• Graduate of an accredited NP or PA program

• Active, unrestricted license in WA, OR, MT, or AK

• National board certification (or board eligible by start date)

• DEA registration and prescriptive authority

• NPI number

• Eligible to participate in Medicare/Medicaid programs

• Current BLS certification

• U.S. citizen or appropriate work authorization

Preferred

• Experience in Skilled Nursing, Long-Term Care, Geriatrics, Internal Medicine, or Family Medicine

• Experience in telemedicine or on-call environments

• Knowledge of CMS and regulatory standards

• Experience managing medically complex or post-acute patients
