Client Care Coordinator

Freedom Home Care is seeking a highly organized, compassionate, and service-driven Client Care Coordinator to join our team at our Mankato headquarters. This vital role serves as the operational heartbeat of our care team—coordinating caregiver schedules, maintaining accurate client and employee data, and acting as a key liaison between clients, families, and caregivers. Key Responsibilities Develop and manage caregiver schedules for new and existing clients, ensuring consistent, high-quality coverage Monitor and adjust daily schedules in real time to accommodate changes, call-offs, and urgent needs Serve as the primary point of contact for day-to-day scheduling communications with clients and caregivers Maintain scheduling accuracy in our home care software system (Axis Care), including caregiver logs, shifts, and client profiles Operational Support Ensure timely system updates to support accurate payroll and billing Participate in the after-hours/on-call rotation to provide scheduling support outside of business hours as needed Assist with general office operations, including answering phones, greeting visitors, and supporting administrative projects Client & Caregiver Relations Build and maintain positive, professional relationships with caregivers, clients, and families Provide support and encouragement to caregivers while promoting reliability, accountability, and compassion Actively contribute to a collaborative, mission-driven, and respectful team culture Qualifications High school diploma or equivalent required; additional education in healthcare or business a plus Previous experience in scheduling, healthcare administration, or home care preferred Proficient in Microsoft Office Suite and comfortable using scheduling platforms Excellent verbal and written communication skills Strong attention to detail and ability to manage multiple priorities in a fast-paced environment Professional presence with a positive attitude and strong customer service skills Flexibility and resilience when responding to unexpected scheduling changes and urgent needs Preferred Attributes Initiative and ownership mindset Strong organizational and time management skills Calm, solution-focused problem solver Compassionate, respectful, and dependable Team-oriented with a service-driven heart Compensation & Benefits Competitive salary based on experience Paid Time Off (PTO) SIMPLE IRA retirement plan with company match Health, Dental, and Vision Insurance Professional development opportunities Internal growth and advancement potential Join Our Team At Freedom Home Care, we believe exceptional client care starts with strong internal systems and empowered teams.
