RN – Case Manager, Ambulatory Care – Day Shift – Relocation Candidates Welcome

We are seeking a compassionate and experienced Registered Nurse (RN) Case Manager to provide comprehensive, patient-centered nursing care in an ambulatory care setting in Sitka, Alaska.

The RN Case Manager will assess patients, develop and evaluate individualized care plans, coordinate referrals and follow-up care, provide patient and family education, and collaborate with healthcare providers and interdisciplinary teams to support quality patient outcomes. The ideal candidate will demonstrate strong clinical judgment, care coordination, communication, documentation, and patient advocacy skills.

Key Responsibilities

• Assess patients' health status, needs, and clinical conditions in an outpatient setting.

• Develop, implement, and evaluate individualized patient care plans.

• Coordinate patient care, referrals, follow-up appointments, and transitions between healthcare services.

• Administer medications and treatments as prescribed.

• Educate patients and families about diagnoses, treatment plans, medications, and ongoing care.

• Collaborate with physicians, providers, and interdisciplinary healthcare teams.

• Monitor patient progress and recognize changes in clinical conditions.

• Escalate patient concerns and clinical changes appropriately.

• Maintain accurate and timely electronic health records.

• Support continuity of care and promote positive patient outcomes.

• Follow infection prevention, patient safety, and regulatory standards.

• Participate in quality improvement initiatives and care coordination activities.

• Float to other departments or assignments as needed.

• Maintain patient confidentiality, privacy, dignity, and respect.

Requirements

• Active Alaska RN license is mandatory at the time of submission.

• Current BLS/BCLS certification required.

• Recent Case Management experience in a clinic or hospital setting within the last 2 years.

• Graduate of an accredited nursing program.

• Case Management certification preferred.

• Strong patient assessment and care coordination skills.

• Excellent communication, documentation, and patient education abilities.

• Ability to collaborate effectively with providers and interdisciplinary healthcare teams.

• Ability to adapt to other departments or assignments as needed.

• Willingness and ability to arrange housing and transportation independently.
