Job Summary:
The Community Health Worker (CHW) provides support to children and families by connecting them with healthcare and community resources. This role works closely with interdisciplinary care teams and internal/external agencies to address social determinants of health and improve overall health and well-being.
Responsibilities
- Connect patients and families with appropriate healthcare and social services.
- Assist patients with navigating healthcare systems and available resources.
- Identify social and community needs and provide appropriate referrals.
- Manage resource identification and referral activities.
- Provide social support and promote patient engagement.
- Collaborate with interdisciplinary healthcare teams and community agencies.
- Help reduce healthcare disparities and promote culturally appropriate care.
- Maintain effective communication with patients, families, providers, and community organizations.
