CenterWell Home Health is seeking a dedicated Home Health Physical Therapist to support patients in their homes, helping them regain strength, mobility and independence through personalized therapy. You will plan and administer prescribed PT, assess progress, modify plans with physicians, educate assistants, and document outcomes in the patient record.
