Job Description Concordia Visiting Nurses is a Medicare‑certified home health agency operating in nine Western Pennsylvania counties. We provide skilled nursing, palliative care, and therapy services. Full‑time (float): Bellevue & surrounding areas The Licensed Practical Nurse (LPN) works under the guidance of the Registered Nurse (RN) as a member of a team to give direct nursing care to patients requiring intermittent visits. The LPN collaborates with patient care with the RN, physician, and patient and follows a patient‑specific treatment plan. QUALIFICATIONS Graduate of a state‑approved school with a course in pharmacology Current license in the State of Pennsylvania Minimum of two years experience in patient care Reliable, independent means of transportation; valid driver’s license and insurance DUTIES AND RESPONSIBILITIES Clinical Skills Provides skilled nursing care including teaching and treatments (IV therapy if certified, venipuncture, complex wound care) and health promotion practice Executes preventive, rehabilitative and restorative nursing procedures appropriate for the patient’s care and safety Assesses signs and symptoms, reports reactions to treatments including drugs, changes in patient’s physical, mental or emotional condition to the physician and supervising RN Evaluates the patient and home situation to determine the health teaching required at each skilled visit Instructs and educates the patient and the designated representative regarding care requirements; conducts a comprehensive physical examination and nursing assessment at each skilled visit Documentation Tablet synced within 24 hours Updates medications in the electronic health record ongoingly Addresses all areas of the skilled nurse’s note at each visit, including comprehensive pain assessment as needed Documents all verbal orders from the physician Completes a hospital hold coordination note as soon as aware of the transfer Documents all communications with the physician and interdisciplinary team in the patient’s electronic health record Benefits 403(b) Retirement Plan Paid Life Insurance Policy Direct Deposit Tuition Reimbursement Program Vacation/PTO Paid Holidays Competitive Wages Referral Program Seniority/Appreciation Bonuses Voluntary Benefits Employee Assistance Program EOE.
