Registered Nurse (RN) Case Manager – HospiceLocation: Forsyth County, NCPosition SummaryAt the heart of hospice is compassionate, patient-centered care—and our Registered Nurse (RN) Case Managers make that possible every day.We are seeking a dedicated and experienced Hospice RN Case Manager to join our team serving Forsyth County and the surrounding communities. In this role, you will provide expert clinical care, coordinate services across the interdisciplinary team, and build meaningful relationships with patients and families during one of life's most important journeys.As the primary nurse managing your patient caseload, you will perform comprehensive assessments, develop individualized plans of care, manage symptoms, provide education and emotional support, and advocate for each patient's comfort, dignity, and quality of life. This is an opportunity to practice nursing with autonomy while making a lasting difference in the lives of patients and their loved ones.Primary ResponsibilitiesComplete initial, comprehensive, and ongoing nursing assessments of patients and family/caregiver needs.Develop, implement, and revise individualized plans of care in collaboration with the hospice interdisciplinary team, patient, and family, incorporating evidence-based palliative nursing interventions.Provide skilled nursing care in accordance with the established plan of care while promoting comfort, dignity, and quality of life.Assess, monitor, and manage pain and other symptoms through clinical interventions and collaboration with the hospice physician.Communicate patient status, changes in condition, and response to treatment with attending physicians and hospice medical directors, obtaining physician orders as appropriate.Educate patients and caregivers regarding disease progression, medications, symptom management, safety, and the end-of-life process.Collaborate with physicians, social workers, chaplains, hospice aides, therapists, volunteers, and other members of the interdisciplinary team to provide holistic physical, emotional, psychosocial, and spiritual care.Maintain timely, accurate, and complete clinical documentation in the electronic medical record.Participate in interdisciplinary team meetings and contribute to patient care planning and coordination.Demonstrate professionalism, compassion, accountability, and respect in all interactions with patients, families, referral partners, and colleagues.Support and uphold the organization's mission, vision, and values while delivering exceptional hospice care.QualificationsEducationGraduate of an accredited school of nursing required.Bachelor of Science in Nursing (BSN) preferred.Licensure & CertificationsCurrent, unrestricted Registered Nurse (RN) license in the State of North Carolina and/or multistate license.Current American Heart Association (AHA) Basic Life Support (BLS/CPR) certification.Valid North Carolina driver's license.Reliable transportation with current automobile insurance meeting state requirements.Experience:RequiredMinimum of one (1) year of experience as a Registered Nurse.Strong clinical assessment and critical thinking skills.Experience developing and managing individualized patient care plans.Ability to work independently while effectively managing a patient caseload.Excellent communication, organization, and documentation skills.PreferredPrevious hospice, home health, palliative care, oncology, ICU, medical-surgical, or other relevant nursing experience.Experience providing end-of-life care and patient/family education.Experience with pain and symptom management.Experience using Homecare Homebase (HCHB) or other electronic medical record (EMR) systems.Knowledge, Skills & AbilitiesStrong understanding of hospice philosophy and palliative care principles.Exceptional assessment, clinical decision-making, and care coordination skills.Ability to build trusting relationships with patients, families, and interdisciplinary team members.Excellent verbal and written communication skills.Demonstrated emotional intelligence, compassion, and professionalism when supporting patients and families through end-of-life care.Comfortable using tablets, smartphones, Microsoft Outlook, Teams, and electronic medical records for documentation and communication.Flexible and adaptable in a fast-paced environment with changing patient needs, census, and service territories.Ability to prioritize multiple patient visits while maintaining quality, compassionate care.Physical RequirementsAbility to stand, walk, bend, reach, lift, and assist patients as needed throughout the workday.Ability to travel throughout the assigned service area to provide in-home patient care.Visual acuity and observational skills necessary to perform comprehensive patient assessments.Work EnvironmentFrequent travel between patient homes and healthcare facilities within the assigned territory.Exposure to infectious diseases and other healthcare-related hazards consistent with patient care.Work is performed in a variety of home and community settings.ScheduleFull-TimeMonday–FridayParticipation in an on-call rotation, weekends, and holidays as assigned.Why Join Our Team?Hospice nursing is more than a career—it's an opportunity to make a profound difference in the lives of patients and families when they need it most.As a member of our team, you'll enjoy the autonomy of managing your own patient caseload while working alongside a supportive interdisciplinary team dedicated to delivering exceptional end-of-life care.
