Pay & Benefits:

• Salary Range: $120,000 $140,000 annually (determined by experience)

What You'll Do:

• Independently perform medical acts within your licensure in outpatient primary care
• Provide holistic care management for individuals, families, and groups, focusing on health promotion and disease prevention
• Collaborate closely with physicians, nurses, and staff to coordinate comprehensive patient care
• Educate patients and families to support informed healthcare decision-making
• Lead clinical assessments to identify health needs and develop care plans
• Support daily clinic operations in a cost-effective, efficient manner
• Participate in ongoing professional education and training to maintain licensure and skills

What You Need:

• Vermont APRN license or eligibility to obtain
• At least 1 year of outpatient primary care experience
• Strong communication and teamwork skills
• Ability to work independently within a collaborative environment
