As a Hospice Nurse Practitioner, you'll play a vital role in planning, organizing, and directing hospice care in patients' homes and communities. Key Responsibilities: Provide medical care, palliation, and manage terminal illness under the supervision of a collaborative physician. Complete examinations, order and interpret test results, diagnose health conditions, and document the findings in the patient’s medical record. Manage health conditions by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or referrals, and coordinating hospice care services. Make recommendations to and consult with the hospice Medical Director. Communicate and collaborate with the interdisciplinary group, the patient’s attending physician, and other community health practitioners. Key Qualifications: Graduate of an accredited nursing school and accredited Nurse Practitioner Program. Current nursing licensure in State and CPR certification. Master's degree with a minimum of (1) year of Nurse Practitioner experience and a minimum of (1) year of hospice or palliative care experience. Accreditation from an approved certifying body for advanced practice nursing as required by the State. Certification in a specialist area is preferred. Excellent observation, verbal and written communication skills, problem solving skills, mathematical skills; nursing skills per competency checklist Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
