Shift Type: Monday-Friday, 8:30 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

• Provide physical therapy treatments in patients' homes, including assessments, evaluations, and plan of care
• Supervise up to 2 PTAs or COTAs
• Complete OASIS documentation and case management
• Collaborate with RN manager and rehab team
• Travel between patient homes within service territory

Qualifications

• CA Physical Therapist License required
• BLS certification (AHA) required
• 2 years experience in healthcare setting, home health preferred
• Current driver's license and auto insurance
• Experience with Homecare Home Base (HCHB) preferred

Knowledge & Skills

• Knowledge of OASIS documentation
• Strong organizational and communication skills
• Ability to work independently in home environments
• Time management and patient scheduling skills
