Long Term Care - LPN
Position Overview As an LPN in long-term care, you provide ongoing chronic disease management, daily nursing care, and routine assessments for residents in assisted living or nursing facilities. The role emphasizes person-centered care, safety, and coordination with a multidisciplinary team to maintain residents' quality of life. Responsibilities
- Monitor vital signs, administer medications, and document responses over time.
- Perform wound care, dressing changes, and routine procedures per care plans.
- Assess changes in condition and communicate with RNs, physicians, and families.
- Assist with activities of daily living and rehabilitation activities as directed.
- Maintain accurate記 records in the electronic health record and ensure safety protocols.
- CPR/BLS
- Experience delivering ongoing care in long-term care settings
- Experience with care planning and documentation in an EHR
- Experience with wound care or palliative care principles
