Key Responsibilities:

• Perform comprehensive physical therapy evaluations for new admissions and provide occasional long-term care evaluations as needed.
• Develop and implement individualized treatment plans.
• Supervise and provide electronic signature review for two Physical Therapy Assistants weekly.
• Collaborate with an interdisciplinary care team to ensure the best outcomes for patients.
• Maintain accurate and timely documentation according to facility and regulatory standards.
• Ensure patient safety and provide patient and caregiver education on therapy plans and exercises.

Requirements:

• Graduate of an accredited Physical Therapy program (DPT or equivalent degree).
• Active Physical Therapist license in the state of New Mexico.
• Current CPR certification, recent TB test, and ability to pass a drug screen.
• Ability to lift and support up to 50 pounds safely, using proper body mechanics.
• Passion for providing exceptional care to the geriatric population.
