Registered Nurse (RN) - Case Management

Registered Nurse Ambulatory Care

Schedule: Monday to Friday, Day Shift

Assignment Details:

• Contract Length: 15 weeks
• Guaranteed Hours: 40 per week

Requirements:

• Current full, unrestricted Registered Nursing license in Alaska or other U.S. state required.
• BLS certification required.
• Case Management experience required.
• Case Management certification preferred.
• Graduate of an accredited school of nursing.
• High school diploma or equivalent required.

Roles & Responsibilities:

• Provide comprehensive, patient-centered nursing care in an outpatient clinic setting, utilizing the nursing process to assess, plan, implement, and evaluate care.
• Administer medications and treatments as per provider orders and policy, ensuring patient safety and adherence to regulatory standards.
• Recognize changes in patient conditions and initiate appropriate interventions or escalate care when necessary.
• Collaborate with providers and interdisciplinary team members to coordinate patient care and facilitate follow-up.
• Document assessments, interventions, and outcomes accurately and in a timely manner in the electronic health record.
• Deliver patient and family education regarding care, medications, and discharge needs to promote optimal health outcomes.

Compliance / Notes:

• Must have active Alaska license at time of submittal; agency staff must have an active Alaska license.
• SEARHC does not offer housing or transportation; it is the employee's responsibility to arrange these.
• Start date will be no less than 5 weeks after traveler's fingerprint cards are received for processing.
