Responsibilities Hickory Trail Hospital (a UHS facility): Hickory Trail Hospital, a 86-bed behavioral health facility located just south of Dallas, has been providing quality health care to the residents of DFW since 1988. Hickory Trail Hospital provides inpatient and day treatment for children, adolescents, adults, and senior adults. Our professional staff individualizes care to uniquely fit the intensity and severity of each client’s case and their family’s needs. Because crisis occurs at all times of the day and night, our services are provided 24 hours a day, 7 days a week. Our Outpatient program (“Better Days” Day Hospital PHP/IOP) offers treatment for adolescents and adults Monday - Friday. For more information about Hickory Trail Hospital, please visit our website at Position Summary: The Discharge Planner is responsible for planning and coordinating activities related to discharge planning. Developing and implementing a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care. No supervisory function. This position also serves in the role of Court Liaison which will coordinate and complete all documentation with the Mental Illness Courts regarding OPC’s, MIW’s, APOWW’s, and court ordered medications. Qualifications Education: Master’s Degree from an accredited college or university, in Social Work or a clinical related mental health field preferred. Experience: Minimum of three (3) years’ experience in a psychiatric health care facility with direct experience in discharge planning. Additional Requirements: Crisis Prevention Trainings (CPI) within 90 days of employment and prior to assisting with a restraining procedure.
