We are seeking a dedicated RN Case Manager for an in-home hospice care role in Glendale, AZ . Position Details: Location: Glendale, AZ Company: Advanced Health Care Type: Field role Responsibilities: Assess patient conditions and develop tailored care plans. Maintain detailed medical records and communicate patient status. Serve as the primary liaison among patients, families, and healthcare providers. Educate patients and families on health management and preventive measures. Coordinate with healthcare professionals to ensure continuous care. Requirements: Active RN license in Arizona with no restrictions. At least 1 year of experience in in-home healthcare. Proficient in case management software, with strong organizational and communication skills. Empathetic, detail-oriented, and autonomous in field settings.
