Benefits: Medical, prescription, behavioral health, dental, and vision coverage 401(k) with 100% match on 3%–9% of pay, based on years of service Employee Stock Purchase Plan with 10% discount Life and disability coverage Flexible spending and supplemental health plans Tuition and education assistance Student loan and certification support Paid time off and family leave Employee Health Assistance Fund Family-building and adoption assistance Counseling and wellbeing resources Moving assistance and additional employee discounts Key Responsibilities: Days: 8:00 AM – 4:30 PM, with 1 weekend every 4 weeks Evenings: 12:00 PM – 8:30 PM with rotating weekends Alternative Evening Schedule: 11:00 AM – 7:30 PM, Monday–Friday; weekday-only roles may rotate weekends as needed Conduct comprehensive assessments of patients' medical, psychosocial, and discharge needs. Develop, coordinate, and document individualized case management plans of care. Collaborate with interdisciplinary teams to manage patient care, level of care, and resource utilization. Conduct interdisciplinary team meetings and coordinate appropriate interventions. Evaluate admissions and continued stays for medical necessity using approved criteria. Assess observation patients for appropriateness of observation status. Perform utilization management reviews and communicate with third-party payors. Coordinate discharge planning and identify barriers to patient throughput. Facilitate appropriate referrals to post-acute services, community resources, and payer case management programs. Serve as a liaison between physicians, patients/families, hospital staff, payors, and outside agencies. Monitor quality, compliance, patient safety, and regulatory requirements. Track and trend barriers and variances to care and recommend process improvements. Identify high-risk patients and coordinate appropriate interventions and referrals. Support patient education, assistance programs, and community-based resources. Promote cost-effective care without compromising patient safety or quality. Maintain accurate documentation of case management interventions and recommendations. Participate in performance improvement initiatives and mandatory education. Report patient safety concerns and significant medical errors according to established procedures. Perform other duties as assigned. Key Requirements and Technology Experience: Minimum 3 years of recent Case Manager experience in an acute care setting, with experience within the last year. Candidates with 3 years of current experience in Med/Surg, Telemetry, Neuro, ICU, PCU, or ED may also be considered. Candidates with Case Management experience in Home Health or Insurance may be considered if they have: 3 years of total acute care experience, and At least 1 year of acute care experience within the last 5 years. Associate Degree in Nursing (ADN) or Nursing Diploma required. Current Florida RN license or appropriate compact RN license required. If holding a compact license, an active Florida RN license must be obtained within 90 days of hire. Advanced Practice Registered Nurse (APRN) license may be accepted if current and compliant.
