Job Summary:
Physical Therapist (PT) specializing in Skilled Nursing Facility (SNF) services is needed in a day shift setting. This clinical role involves partnering with residents and care team members to maximize functional abilities and independence through evaluative and therapeutic procedures. The position emphasizes resident-centered care and interdisciplinary collaboration to promote optimal outcomes.

Job Details:

• Schedule: Day shift, 8 hours per day, with a minimum of 40 hours per week.
• Required Qualifications: Valid license as a Physical Therapist (PT) in the applicable state of practice. Bachelors degree in Physical Therapy is required; Masters or Doctorate degree is preferred. Basic Life Support (BLS) certification is preferred. No minimum years of experience specified. Experience in a long-term care or skilled nursing facility is preferred.
• Preferred Qualifications: Prior experience working in a skilled nursing or long-term care setting. Membership in professional organizations and ongoing professional development are advantageous.
• Additional Information: The therapist will perform evaluative and therapeutic procedures under physician or healthcare provider orders, develop and modify treatment plans, and instruct residents and caregivers in the use of assistive devices. Responsibilities include documentation in electronic medical records, collaboration with therapy professionals, supervision of assistant-level staff when appropriate, and compliance with Medicare, Medicaid, and other reimbursement regulations. Ensuring departmental cleanliness, maintaining licenses, certifications, and competencies, and providing education to staff are also key duties.
