Integrated model of care to adults who reside in skilled nursing and assisted living facilities. Locations available in the Dayton andCincinnati areas.

• 18-22 patient patients per day (20 patients per day = about $295,000)

• Unlimited PTO: 4% of salary is set aside and can be used toward PTO or can be kept.
